Offshore workers were put at risk when a supply ship crashed into a platform in the North Sea at high speed.
The Health and Safety Executive (HSE) has reported that a ship entered the safe zone around Apache’s Forties Echo platform at “excessive speeds” on March 16.
The Sea Falcon vessel then collided with the platform, causing “significant” damage.
Following the collision, 15 men were airlifted from the platform, 110 miles north-east of Aberdeen.
The HSE report said: “The Sea Falcon entered the 500 metre safety zone around the Forties Echo installation and traversed he zone at excessive speeds above four knots on a heading directly towards the installation.
“The Sea Falcon collided with the installation, causing significant topside damage to the installation and the vessel.”
The owners of the Sea Falcon, Singapore-based Deep Sea Supply, were told that they had “failed to ensure the health and safety of employees and others from the risk of injury.”
Deep Sea Supply has since taken preventative measures to stop similar incidents in the future.
